Do you have cancer?
Applicant must be diagnosed with cancer as defined by the patient’s oncologist.
Do you live in the Coachella Valley?
Applicants are to reside and receive their treatment in the Coachella Valley: Palm Springs, Palm Desert, La Quinta, Indian Wells, Rancho Mirage, Cathedral City, Indio, Coachella, Mecca, Sky Valley, or Desert Hot Springs.
